If you have received an invoice from ACAC for application fees, examination fees, recertification fees or reinstatement fees, follow these simple steps to keep your certification current.

1. Print and complete this one-page form.  (If you are an entry-level certificant applying for additional recertification as an employee or apprentice, click here to download the correct form.)

2. Enclose a check for the required fees payable to ACAC.

3. Mail to ACAC at PO Box 1000, Yarnell, AZ  85362.
